Sun-Kissed Guidelines: Your Guide to Responsible Tanning at Bel-O-Sol

I, the undersigned customer, agree to release and hold harmless Bel-O-Sol Tanning Salon (operating under 1427141 Ontario Inc.) and its staff, agents, and affiliated parties from any liability related to injuries or damages incurred on the premises or as a result of using the equipment and services provided.

I acknowledge that I have had the opportunity to consult a physician concerning any health risks associated with services provided by Bel-O-Sol Tanning Salon. By providing my email and phone number, I consent to receive marketing communications via email and text message.

I understand that Bel-O-Sol will not be held liable for any skin sensitivities, allergies, or other medical conditions that may arise from the services. The salon is also not responsible for any loss or theft of personal property. I agree not to disparage Bel-O-Sol Tanning Salon or its affiliates in any public forum or social media.

I will immediately discontinue any services should I experience skin irritations, allergic reactions, or other medical conditions. I confirm that I will use protective eyewear and other safety measures while using tanning equipment and that I have consulted with a healthcare provider regarding such usage.

I acknowledge that:

• Stored items (lotions/goggles) may be discarded 12 months after my last visit.
• Tanning minutes, packages, and Versa Spa sessions expire 365 days from the date of purchase.
• All sales are final; no refunds will be issued.

By signing below, I agree to all terms and conditions and confirm that I am aware of the risks associated with UV exposure, acknowledging that I am tanning at my own risk.
